• You may print and fill out your forms in advance of your next appointment.
   This will expedite the registration process.

• There is no need to contact our office for your prescription refills, which take 48 hours
   to process, excluding weekends. Instead, contact your pharmacy directly for your refill
   needs and they will contact us.

• Our office is open Monday through Friday from 9:00 am till 5:00 pm.
   We are closed from 12:00 pm till 1:00 pm daily.
